Everything the liver does

All the substances released from the gut, pancreas and stomach reach the liver, are metabolised, and are then sent on to the rest of the body. The liver regulates the body's supply of nutrients and filters out harmful substances from the gut.

It produces essential proteins, maintains homeostasis, and handles coagulation, digestion, glucose balance and protein metabolism. It makes glutathione for detoxification and to protect organs from oxidative stress, and it breaks down medicines — and alcohol, toxic in its original form — into a form the body can tolerate.

Why fatty liver is now almost everywhere

It is no longer unusual to find fatty liver in almost every routine scan — it is now the world's most common liver condition, affecting an estimated one in three adults, and it is climbing fast in India across all ages, including the young and the outwardly slim. The reason is our modern way of living: diets heavy in sugar, refined carbohydrates, processed food and fructose, combined with sedentary days, overwhelm the liver's capacity to handle fat, so it begins to store it inside its own cells.

This is MASLD — metabolic dysfunction-associated steatotic liver disease, formerly called NAFLD (non-alcoholic fatty liver disease). It is far more than a cosmetic finding on a report: it is the liver's expression of a wider metabolic problem, and it travels hand in hand with insulin resistance, obesity, type 2 diabetes and heart disease, raising the risk of all of them.

A silent organ — the danger of no symptoms

The liver's silence is precisely what makes fatty liver so dangerous. Most people with MASLD feel completely well and have no symptoms, or only vague ones — mild fatigue, a heaviness in the upper right abdomen — so the condition is usually discovered by accident, if at all. Liver blood tests can even read normal while fat and inflammation quietly build.

Because of this, damage typically progresses unnoticed for years. By the time obvious symptoms appear — jaundice, swelling, easy bruising, confusion — the disease has often reached an advanced stage. This is exactly why acting early, while the liver can still fully recover, matters so much.

From fat, to inflammation, to scarring

In many people, simple fat in the liver progresses to MASH (metabolic dysfunction-associated steatohepatitis, formerly NASH): fat plus active inflammation. This is the turning point. Chronic inflammation injures liver cells and, in trying to repair itself, the liver lays down scar tissue — fibrosis — much as a wound heals with a scar.

As inflammation continues, that scarring spreads and stiffens, and healthy liver tissue is steadily replaced by scar. Left unchecked over years, this is the road that leads to cirrhosis — and, in some, to liver cancer. The driver at every stage is the same low-grade inflammation that underlies so much modern chronic disease.

Cirrhosis — increasingly common, not always the end

Cirrhosis is advanced, widespread scarring that hardens the liver and impairs its function, and it is becoming far more common as fatty liver and alcohol use rise — increasingly in people younger than ever before. Even here the liver can stay silent for a long time, showing only fatigue, nausea and weight loss before more serious signs appear.

Yet a diagnosis of cirrhosis is not automatically a dead end. In its earlier, compensated stages the liver still functions, and its remarkable regenerative capacity means that removing the causes and calming the inflammation can halt progression, and in some cases allow real recovery of function — buying patients years of better health, and quality of life.

The liver in Ayurveda

Ayurveda holds that the liver — closely tied to Ranjaka Pitta and Yakrit — converts Rasa Dhatu into Rakta Dhatu, and identifies and detoxifies Ama (toxins) in the Rasa Dhatu before it can enter the blood, keeping the blood pure. This is the main reason it places such emphasis on maintaining healthy liver function.

When the liver is sluggish, Ama accumulates and may enter the blood, driving inflammation; toxins stored in the liver can lead to allergies, high cholesterol, disturbed blood sugar, constipation, digestive problems and fatigue. Left long enough, serious liver disease develops. Ayurveda's answer is to rekindle the liver's fire, clear the Ama and restore free flow — treating the cause, not merely the report.
